6) सभी शैक्षणिक प्रमाण पत्र की छायाप्रति स्व-अभिप्रमाणित 

7) पात्रता/दक्षता परीक्षा प्रमाण पत्र

8) फोटो 5

9) आधार कार्ड की छायाप्रति

10) पूर्व से नियोजित शिक्षक हैं तो सक्षम प्राधिकार से निर्गत अनापत्ति पत्र, 

संबंधित विद्यालय से विरमण पत्र एवं विद्यालय प्रधान से बकाया, आरोप आदि

विद्यालय में योगदान के लिए अपने प्रधानाध्यापक / प्रधानाध्यापिका

के नाम पत्र का फार्मेट

            उपर्युक्त विषयक प्रसंग अंकित करना है कि मेरा चयन

 बिहार राज्य विद्यालय अध्यापक (नियुक्ति स्थानांतरण, अनुशासनिक

 कारवाई एवं  सेवा शर्त) नियामावली 2023 के नियम 9 (i) के

 आलोक में कार्यालय, जिला शिक्षा पदाधिकारी (जिला का नाम) 

 के पत्रांक-22/2024 एवं दिनांक .......के आलोक में (विद्यालय का नाम)

 में 1-5/6-8/9-10/11-12 के शिक्षक के पद पर हुआ है उक्त आदेश

 के आलोक में मैं आज दिनांक .../05/2025 के पूर्वाहन/अपराह्न में 

विद्यालय में योगदान हेतु उपस्थित हुआ हूंl
